Details - Skilled Nursing Facility (SNF) setting, encompassing inpatient, long-term care, and short-term rehabilitation - Primary patient population includes geriatric residents with mobility limitations, functional deficits, and varying levels of independence - Day shift schedule, 9AM–5PM Requirements - Graduate of an accredited Physical Therapy program - Current, active state PT license required; license required if applicable - CPR certification required or preferred depending on facility policy - Experience in a SNF or geriatric rehab setting preferred - Strong communication, documentation, and interpersonal skills - Ability to work independently and as part of a multidisciplinary team - Must be able to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility Additional Information - Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, and delivering evidence-based PT interventions targeting strength, coordination, balance, and mobility - Collaborates closely with physicians, nursing staff, occupational and speech therapists, and family members to coordinate comprehensive patient care - Required to monitor and document patient progress in compliance with state, federal, Medicare/Medicaid, and facility guidelines - Participates in care planning meetings and interdisciplinary team conferences - May supervise PTAs and therapy aides as needed - Patient and caregiver education on safe mobility techniques, home exercises, and adaptive equipment is a key component of the role American Traveler Job ID P-680110.
