Our treatment includes multiple modalities as needed such as medication support, psychotherapy, case management, mobilization of community resources, crisis prevention, and crisis management to prevent higher levels of care. Pathways to Wellness supports the Health and Wellness model of care. Our group is also committed to employing the evolving models of behavioral health integration in primary care. We are seeking a new Psychiatric Mental Health Nurse Practitioner to join our team. Your role will be to provide psychiatric services for our clients with moderate to severe mental health impairments. As a member of the clinical department, you will work with Psychiatrists, Physician Assistants, Clinical Pharmacists and Nurses and report to our Program Manager and Physician Advisor. What You Will Do: Initial psychiatric evaluations of patients. Formulate an initial diagnosis and treatment plan for individuals with common mental health problems. Provide assessments, crisis response, medication and medication injections, direct mental health services, and medical record documentation, as required; Management and coordination of patient care, to develop the most appropriate medication regimen given the patient's history and presenting symptoms; Work closely with patients and multi-disciplinary teams to help enhance medication adherence through patient education, one-to-one and/or group interventions with the patient, and monitoring to ensure compliance with the medication procedures that are in place; What You Will Bring: Our ideal candidate will have a minimum of 2 years of relevant mental health experience and the following credentials/skills: Board-Certified Psychiatric Mental Health Nurse Practitioner/PMHNP Current DEA Number ANCC NPI # and ability to obtain current Medicare and Medi-Cal number.
