Occupational Therapist Location: San Ramon, CA Agency: Windsor Healthcare Recruitment Group, Inc. Pay: $2,754 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 26 Weeks Start Date: ASAP About the Position NEW Grads Welcome CVHCare is looking for a qualified Occupational Therapist. Home health experience preferred not required. Assigned county Territory is Area Area 2 Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont and Area 8 Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington TWO Options - Guaranteed Hours or Non Guaranteed hours. If Guarantee, the following shall apply: Productivity: At least 27 points per week (points described below). 36 hours Guarantee. Traveler must complete 30 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients, guarantee does not apply. Guarantee does not apply for 1st and last week of assignment, or for weeks in which there are holidays, or for weeks in which the traveler has time off. If Non-Guarantee, the following shall apply: Weekend coverage may be required. To secure a full caseload, Traveler may be required to go outside the service area occasionally. Can refuse patients. No guaranteed hours. Traveler must be willing to see Covid and Covid Pui patients. Payment made on per visit basis. See below for visit types and how they convert to points and how they convert to hours. This is how we will calculate how many hours are approved per day. Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours. OASIS ReCertification and OASIS RoC = 1.5 point = 2 hours. Although Traveler s are paid hourly by their agency, hours are approved by CVHCare on a per visit bases. Note that Rate is all inclusive and covers drive/travel time, visit time, documentation time, & time spent in making phone calls to patients & doctors. Credentials: Current CA OT License, BLS, Drivers License, Auto ins, Auto Reg, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical. Your Agency s contract to your traveler MUST mirror the confirmation page. If your agency does not comply with this standard, your agency will be at risk of being removed from this client s vendor set. No exceptions! Start Date Must be Identical. End Date Must be Identical. Please Note - All Requested Time Off (RTO) must be disclosed at the time of submission. If your agency or the traveler does not include all RTO in the submission form and adds it post confirmation, your agency will be at risk of being removed from this client s vendor set. No exceptions! Time off requested after the acceptance date, will not be approved.
