Job Description:
Job Description Job Description Salary: 100,000 Summary: The
Behavioral Health Provider (BHP) will provide direct services to
MCHWC patients and community members in the integrated care model,
closely working with PCPs and other medical and non-medical staff
involved in patients care. The BHP understands and practices
techniques in the integrated model, including time-limited and
evidence-based intervention and therapy appropriate for the primary
care setting. The BHP may work at multiple MCHWC sites. Essential
Duties and Responsibilities: Provides direct patient services for
both MCHWC patients and individuals referred from the community
seeking to establish care with MCHWC. The BHP has experience and
knowledge in treating a variety of DSM 5 conditions (generalist)
and patients of all ages. Conducts brief interventions with a high
volume of patients via several different visit types (e.g.,
individual, group, co-visits, etc.) as well as provide
consultation/ recommendations back to the primary care provider or
team. Provides brief (30 minute) and time-limited (4-8 visits)
behavioral, psychoeducational, or cognitive behavioral
interventions for patients. Maintains a visible presence and is
available for same day and scheduled initial consultations with
patients referred by PCPs. Effective and rapid triage as it relates
to BH services, and referral to other mental health providers or
higher levels of care when necessary. Assists primary care
providers in recognizing and treating behavioral health and
psychosocial problems through the integrated model. Provides PCPs
or team members with verbal or written feedback documented in the
EHR on patient encounters. Provides expertise on behavioral health
problems and issues for MCHWC staff through in-service trainings,
consultations, and other informal means. Demonstrates effective
collaboration and problem-solving skills. Consistently attend and
actively participate in all BH department and clinic meetings Other
duties and/or special projects as assigned by Director which are
consistent with the position and in compliance with BH program and
agency policies and procedures. Assists in the development of
patient education materials, clinical pathway programs, group
shared medical appointments, classes, and behavior focused practice
protocols as requested. Completes timely documentation and other
necessary paperwork complying with all regulations. Provides
patient care utilizing critical thinking skills to analyze social,
safety, environmental, psychological and spiritual issues for the
patient. Assists in the setting and attaining program goals and
objectives Assesses, reports, and provides intervention in cases of
domestic violence, child, dependent adult, or elder abuse, and
other reportable situations in accordance with ethical practice,
legal mandates, and the health centers policies Receives feedback
in a professional manner and implements feedback received
Participates in program development and quality improvement and
assurance activities as needed Performs other duties as directed,
developed, or assigned. Supervisory Responsibility:N/A
Qualification Requirements: Education and/or Experience: Licensed
Clinical Psychologist or LCSW Language Skill: Must possess
excellent organization, writing, and verbal skills English
proficiency required Reasoning ability: Ability to interpret a
variety of instructions furnished in written, oral, diagram, or
schedule form. Equipment/Machinery:Fax machine, copier, personal
computer, telephone, calculator, Microsoft Word/Excel, EHR system,
and other software as required. Physical Demands: The physical

Stand and walk or sit alternatively depending on specific needs of
day. Estimate 20% of time is spent on feet and 80% sitting at desk.
Have occasional need to perform the following physical activities:
bend/stoop/squat, climb stairs, push or pull, reach above
shoulders. Have occasional need to perform standing and walking
activities. Constant need to perform the following physical
activities: writing/typing, grasping/turning, finger dexterity.
Lifting/carrying over 10 pounds occasionally. Lifting/carrying less
than 10 pounds frequently. Vision requirements: constant need to
complete forms, read reports, view computer screen. Frequent need
to see small detail. Frequent need to see things clearly beyond
arm's reach. Hearing requirements: constant need to communicate
over telephone and in person. Travel Requirements: Occasional need
to utilize personal transportation to visit sites, and attend
meetings and conferences.
