Job Description LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Lodi, California. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Duration: 13 weeks - 40 hours per week - Shift: 10 hours, days - Employment Type: Travel Job Summary: We are seeking an experienced Registered Nurse (RN) – Care Manager with a background in acute care management. The ideal candidate will coordinate patient care, manage treatment plans, and ensure efficient utilization of healthcare resources while maintaining high standards of patient outcomes. Key Responsibilities: - Coordinate and manage patient care plans across the continuum of care - Collaborate with physicians, nurses, and interdisciplinary teams - Assess patient needs and develop appropriate discharge plans - Ensure effective utilization of healthcare services and resources - Monitor patient progress and adjust care plans as needed - Maintain accurate documentation and compliance with hospital policies Required Qualifications: - Active Registered Nurse (RN) License – State of Practice (California) - Minimum 1 year of acute care management experience - Strong clinical assessment and care coordination skills - Ability to work flexible schedules, including weekends Required Certifications: - RN License (California) Preferred Qualifications: - Bachelor’s Degree in Nursing (BSN) - Experience in a Care Management / Case Management role - Case Management Certification (CCM or equivalent) About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
