Village II Property Type: Senior Case Manager II The Case Manager
II plays a vital role in MidPen's Resident Services team,
supporting residents in supportive housing to maintain stability
and achieve self-sufficiency. This role is essential to MidPen's
mission of providing high-quality, affordable housing and fostering
thriving communities. Reporting to the Resident Services Manager,
the Case Manager II works closely with residents, property
management, and community partners to deliver impactful services.

and resident well-being. Responsibilities: Deliver onsite
programming and direct case management services to residents at
high-complexity supportive housing sites Conduct outreach through
door-to-door, phone, and email to engage residents in available
services Provide crisis intervention and follow-up, including
mandated reporting for child or adult abuse Facilitate individual
and group services that promote independent living, wellness, and
recovery Collaborate with residents to set and achieve personal and
professional goals Manage and evaluate services provided by
third-party partners and community agencies Partner with Property
Management and Resident Services teams to address resident concerns
and support community building Qualifications: You support
residents in achieving housing stability and personal growth
through direct services and community partnerships. M.S.W. or M.A.
in psychology, counseling, or related field with 2 years of
experience serving high-needs populations, or B.A./B.S. with 3
years of experience Experience working with individuals who have
experienced homelessness or trauma Familiarity with mental health
diagnoses and interventions including harm reduction and
trauma-informed care Strong communication skills and ability to
navigate language barriers using translation tools Proficient in
Microsoft Office Suite, Salesforce, and virtual collaboration tools
Valid California Driver's License, current auto insurance, and
reliable transportation Ability to work occasional evenings and
weekends and travel between properties Education and Experience:
M.S.W. or M.A. degree in psychology, counseling, or related field
and 2 years of full-time experience serving high needs populations
(mental illness, substance use, co-occurring disorders) or B.A. or
B.S. degree in the same fields with 3 years full-time experience
serving these populations Experience working with and serving
people who have experienced homelessness Experience working with
high needs or high acuity populations that have experienced trauma
Experience using and administering multiple assessment tools and
interpreting information/data derived from assessments Experience
supporting program and service implementation and coordination
Experience collaborating with local service providers Pay Range
$70,304 - $82,000 Annual Salary - Pay based on applicable
